Valuing People First

Supported living is a way of helping adults with learning disabilities to have the opportunity to live as independently as possible.

We value all people and recognise that each individual is unique. Our Supported Living services provide person centred support to people with learning disability and Autism, enabling them to live independently within their own homes or in shared accommodation. We work in partnership with health and social care professionals such as Speech therapists, Physiotherapists, Social workers, Education and Vocational training providers to enable you to live a healthy and fulfilled life. People living in our supported living schemes are supported to access local facilities which enables them to remain independent and build confidence.

Our Staff and Team

We have a team of staff who have vast experience in working with people with learning disability and Autism including those with complex needs. The staff have been trained in working with people who have behaviour that challenges. Our experienced support workers support all aspects of daily life to develop the skills required to enable you to live more independently. Our Learning Disability Nurse provides specialist support to service users and their families and to support workers working with those who have complex needs.

Our Supported Living services provide person centred support enabling people to live independently within their own homes or in the local community. We use person-centred approaches to assess and identify needs. Care plans are person centred to meet each client’s identified needs.

Activities

We work in partnership with health and social care professionals, housing, education, and vocational training providers in order to achieve the best possible outcomes. Our experienced support workers support all aspects of daily life to develop the skills required to enable you to live more independently. The support workers work with people to engage in meaningful activities of daily living and help to build confidence. The help you may receive includes:

Computer games

Play Station

Walking

Cinema

Bowling/Golf

Who we support:

We have support workers who have received specialist training in supporting people with these conditions and others, who are able to support people with behaviours which may challenge. We will work with other professionals to produce positive behaviour plans.

Accommodation Options

If you are finding it difficult to cope living on your own and do not want to live in a residential setting, you may wish to consider Supported Living.

We can help you to choose any of the following supported living options:
• sharing a house or flat where other people get the same help as you
• having your own house or flat with the support you need

All accommodation we provide is adapted to suit your individual needs and requirements. The Service Manager will support you in making a decision about which option suits you best.

What happens next

When you have decided that you would like to live independently, contact the Service Manager who will be able to discuss your options and explain the process. You can then make an informed decision about what you want. You may wish to have an advocate or a family member to help you decide what you wish to do, but ultimately, we will help you make the final decision about the support you want. When you have moved in, your Service manager will visit you and talk to you to find out if you are happy with how things are going. It is important to talk about anything you are happy or unhappy with. If you decide to move into our supported living scheme, you will be allocated a key worker. You will meet with your key worker once a month to discuss how you are getting on and to raise any concerns if you have any. There is always staff present at all times whom you can approach to discuss any concerns or issues or even improvements you would wish to make to your home.
